This notice covers the sample-request form on staging.evolvaiagents.com. This is an Evolv staging preview, not a checkout or automated delivery service.

What the form stores

We store the business email, website or social link, optional business name and context you provide, together with a request reference, submission time, source page, consent-notice version, and manual-review status. Please do not submit passwords, payment information, patient details, or other sensitive information.

How the information is used

Your details are used to review and discuss the requested sample. Submission does not enroll you in unrelated marketing, connect a social account, publish content, or purchase a service. This form does not automatically send email or create a personalized content week.

Where requests are kept

Requests are held in this staging WordPress database. They are not public pages and are available through an administrator-only request queue. No CRM or social publishing service receives submissions from this form. Existing server access logs may record connection information. A keyed, non-reversible connection identifier is temporarily used to limit repeated requests.

Retention and your choices

Requests remain stored for manual review until an administrator removes them. An automatic retention period has not been established for this preview. If you submit a request, retain its reference. When Evolv contacts you about it, you can reply to request a correction or deletion. Avoid entering real personal information while evaluating this staging site.

Browser technologies

The new homepage and sample funnel do not load third-party analytics, remote fonts, advertising pixels, or social embeds. A short-lived security token is used when submitting the form. WordPress administration uses its own login cookies.
